Glucophage SR Tablets

Glucophage SR tablets belong to the category of metformin sustained-release tablets. Clinically, this medication is primarily used for patients with type 2 diabetes mellitus, particularly for those who cannot achieve good glycemic control through diet and exercise alone.

Indications:

Glucophage SR tablets are recommended to be used in combination with dietary and exercise therapy to control blood sugar levels in patients with non-insulin-dependent (type 2) diabetes mellitus. This medication can also be used concomitantly with sulfonylurea hypoglycemic agents or insulin to control blood sugar levels in patients with type 2 diabetes. To minimize gastrointestinal complications and achieve adequate glycemic control with the smallest dose, patients should start with a low dose and gradually increase the dosage.

Precautions:

1. Transition from Other Antidiabetic Treatments: Except for chlorpropamide, patients usually do not require a transition period when switching from other oral antidiabetic agents to Glucophage SR. However, patients previously taking chlorpropamide should be closely monitored during the first 2 weeks of switching to Glucophage SR, as chlorpropamide has a long elimination half-life and may lead to hypoglycemic episodes due to excessive drug effects.

2. Concurrent Use with Sulfonylureas: If patients do not respond after several weeks of taking the maximum recommended dose of Glucophage SR, consideration should be given to gradually adding a sulfonylurea oral hypoglycemic agent while maintaining the maximum dose of Glucophage SR, unless the patient has primary or secondary failure to sulfonylureas. Currently, there are only clinical and pharmacokinetic data available on the interaction between metformin and glibenclamide (glyburide). By adjusting the doses of both medications, satisfactory glycemic control can be achieved with concurrent use.

The risk of hypoglycemia associated with sulfonylureas persists and may even increase when used concomitantly with Glucophage SR, and appropriate prevention measures should be taken.

3. Concurrent Use with Insulin in Adults: The insulin dose can be maintained when initiating treatment with Glucophage SR. The initial dose of Glucophage SR for patients receiving insulin treatment should be prescribed according to the doctor's instructions. Individualized adjustments should be made based on the glycemic response.
