The main distinctions between indirect inguinal hernia and direct inguinal hernia lie in the age of onset, the protruding shape of the hernia, the route of protrusion, and the treatment methods.
1. Indirect Inguinal Hernia: It primarily occurs when the contents of the intestine break through the peritoneal area and enter the inguinal canal, mostly affecting children and young adults. This results in protrusion through the inguinal canal, which can extend into the scrotum. The hernia sac is mostly elliptical or pear-shaped with a pedunculated appearance. Timely treatment is required through laparoscopic hernia repair surgery.
2. Direct Inguinal Hernia: This occurs when the contents of the intestine penetrate the peritoneal area and enter the femoral canal, predominantly affecting the elderly. The protruding shape is mostly hemispherical with a broad base. The hernia protrudes through the direct inguinal hernia triangle and does not extend into the scrotum. Additionally, direct inguinal hernias rarely become incarcerated, whereas indirect inguinal hernias have a higher incidence of incarceration.
